Position Summary: The GSSC will conduct health and functional assessment for elders enrolled in Senior Care Organizations (SCO). The GSSC will also develop and monitor service plans to promote the client's overall health and independence. The GSSC will work with an interdisciplinary team to assure effective coordination and delivery of care in the client's home. 

Essential Functions: (Core duties or tasks that is fundamental and not marginal to the performance of the job)

  • Perform, arrange, and/or participate in Initial and Ongoing Assessments of health and functional status of Members, including determining appropriateness for institutional long term care services.
  • Developing community-based care plans and related service packages necessary to improve and maintain Member health and functional status.
  • Participating in development of a Member's Individual Care Plan (ICP).
  • Participate as part of a Member's Primary Care Team including SCO RNs and other medical disciplines.
  • Arrange and coordinate the provision of appropriate community long term care and social support services.
  • Monitor the provision and outcome of effectiveness of community services as defined by the Member's ICP.
  • Track Member transfers from one setting to another (hospital to home).
  • Promote independent functioning of the Member and provide services in the most appropriate, least restrictive environment.
  • Attend SCO Clinical Meetings, team meetings and other activities as a member of the GSSC team.
  • Other duties assigned by Managed Care Program Management.

Qualifications: BA/BS in social work, human services or related field required; experience with Elders in a Health Care setting preferred. Experience with care coordination and ability to work within an interdisciplinary team setting required. Strong written, verbal, computer skills required. Bilingual ability in Khmer is required.


Hours: 37.5 per week
