                                  Elder Services of the Merrimack Valley, Inc.

Position Responsibility: Responsible for providing the program overview to interested consumers and to assist members who have obtained prior authorization for PCA services and their surrogates, if necessary, in developing the skills and resources to maximize the member's management of personal health care, personal care services, activities of daily living, and activities related to the fiscal intermediary.

Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)

  1. Perform intake and orientation on new PCA referrals to determine the initial eligibility for services, to instruct the Consumer in the rules, policies and regulations and to describe the role of the person care agency and the fiscal intermediary.
  2. Manage an on-going case load of PCA consumers.
  3. Provide functional skills training to instruct the Consumer/Surrogate in the basic requisites of an effective program of personal care services.
  4. Instruct the Consumer/Surrogate on completing time sheets, understanding the medical issues and limitations of the Consumer and reviewing the responsibilities of the Consumer/Surrogate.
  5. Instruct Consumer/Surrogate on how to hire a new Personal Care worker, review paperwork for new hire and provide a new employee packet.
  6. Perform quarterly evaluations on new Consumer/Surrogate and additional training as needed.
  7. Offer instruction on issues regarding benefits, durable medical equipment and referrals to other programs.
  8. Keep current personal data, progress notes and tracking software information in PCA Program software and case file.
  9. Work collaboratively within an interdisciplinary team of Nurses, Skills Trainers and Case Aides.
  10. Assist participants with Medicaid applications, securing Social Security and S.S.I., and other related benefits. 
  11. Other related duties as assigned.

Qualifications: Bachelor's Degree preferred; minimum 4 years experience in Human Services (preferably with Elders or the Disabled) required. Excellent written, verbal, interpersonal and computer skills necessary. Strong organization skills required. Bilingual capacity in Spanish is required.

 

Hours: 37.5 per week
